Please give the full names of the people in your family, and far as possible the dates and places of births, deaths and marriages. Use one form for each couple and their children: for example, one form for your parents and their children, one for yourself and your spouse and your children, and any other couple and their children. Add extra children on back. If a person had two marriages use two forms, and indicate any divorce. If a couple had children without being married treat them as if they were married but omit the date and place of marriage, and make a note there instead. Sources of information: If you have certificates of birth, marriage or death please indicate, and otherwise note the sources of your information. It might come from personal experience, or your parents might have told you or you might have a family Bible, or letter. Please give whatever information you can even if you do not have documents.
